Resolved... This turned out to be issue in the MacOS settings... System Preferences > Security & Privacy > Microphone > REW somehow REW had become unchecked. Once I rechecked the box for REW, the UMIK-1 started working properly again in REW.

I've confirmed that the USB is firmly connected into the Mic and have tested it on both USB ports on my MacBook... Blue light on the Mic is on... Not 100% sure how the Audio MIDI Setup should be configured, so included a screenshot... Also included the screenshot of the REW sound card...
